A Low Ferritin on a Vegetarian or Vegan Diet

Plant iron is absorbed far less efficiently than iron from meat, and what you drink and eat alongside it changes how much gets in by a large factor. So the practical lever here is timing and combination, not eating more iron-containing food. The trap is letting the diet stand as the automatic explanation and skipping the tests anyone else would get.

Why the numbers look like this

Iron reaches the body by two routes. Haem iron, found in meat and fish, has its own transporter and is absorbed relatively efficiently regardless of what else is in the meal. Non-haem iron, which is all that plants provide, has to be released and reduced before it can cross the intestinal wall, and that step is easily disrupted.

Tea and coffee are the largest interferences and the most common. Polyphenols in both bind non-haem iron and can cut absorption dramatically, and a cup with a meal or shortly after it does most of the damage. Calcium, whether from dairy or a supplement, competes directly. Phytates in wholegrains, legumes and nuts bind iron, which is why the foods highest in plant iron also carry the strongest inhibitor.

Vitamin C works the other way and powerfully. It keeps iron in the form that can be absorbed, so an acidic food or drink alongside a meal can raise absorption several-fold.

Taken together, this is why estimated iron requirements run substantially higher for people eating no meat. The gap is about absorption, not about how much lands on the plate.

Not being flagged is not the same as normal

The lower limit printed for ferritin sits below the level at which symptoms commonly appear, and that matters more here than usual, because tiredness gets put down to the diet instead of to the deficiency. A value in the lower part of the range in someone with symptoms is worth acting on. Any inflammation lifts ferritin, so a result taken while unwell can read as adequate with the stores already low.

What else on the report can hide this

B12 belongs with every iron result in this group, since a vegan diet provides essentially none and the deficiency causes irreversible neurological damage if it goes long enough. It is not optional and it is frequently omitted.

Celiac serology, requested with a total IgA so a negative result can be trusted, is the test most often skipped once a dietary explanation is available. Celiac disease is common, it causes exactly this, and vegetarians get it at the same rate as everyone else.

Outside the menstruating years the same rule applies as anywhere else: iron deficiency prompts a search for a source of bleeding, and a plant-based diet is not a reason to skip it.

Menstrual loss should be quantified and not assumed ordinary, since it is the dominant cause in menstruating people whatever they eat.

The dietary advice worth giving is specific rather than general: separate tea and coffee from meals by an hour or two, take calcium supplements at a different time, and pair iron-containing foods with something acidic. Those changes move absorption more than adding another iron-rich food does.
