A Raised TSH in Pregnancy

Pregnancy uses different thyroid reference ranges from the rest of medicine, and a result read against the ordinary range is read wrongly. Requirement also rises early, often before the first antenatal appointment, so a woman already on levothyroxine usually needs more of it from the first weeks, not from the first scan.

Why the numbers look like this

Pregnancy raises the demand for thyroid hormone through several channels at once. Estrogen increases the binding proteins that carry the hormone, so a larger total pool is needed to keep the free fraction steady. The placenta degrades thyroxine as part of its normal work. Most importantly, the fetus cannot make its own thyroid hormone until roughly mid-pregnancy and depends on the maternal supply for brain development until then.

A healthy gland scales up and the numbers barely move. A gland already under autoimmune attack, or a fixed levothyroxine dose set before conception, cannot scale, and the TSH climbs as the pituitary asks for output that is not coming.

Not being flagged is not the same as normal

Trimester-specific reference ranges are how this result is read at all: the first-trimester upper limit for TSH sits below the ordinary one, so a value that would pass outside pregnancy can be genuinely raised inside it, and the reverse misreading happens too. Positive TPO antibodies lower the threshold at which treatment is considered, which makes the antibody result part of the interpretation rather than an extra.

What else on the report can hide this

TPO antibodies predict both progression during the pregnancy and thyroiditis after delivery, so they change the monitoring plan either way. The practical interactions matter as much as the biology: pregnancy vitamins are built around iron and calcium, both of which bind levothyroxine in the gut, and severe vomiting removes the tablet before it can be absorbed at exactly the stage when demand peaks. Free T4 assays also become less dependable as binding proteins rise, which is why the TSH, read against the right trimester range, carries the weight here.

What usually causes it

Listed from most to least common — not from most to least serious.

  1. Very common

    Autoimmune thyroiditis unmasked by pregnancy

    A gland that coped before cannot meet the increased demand. TPO antibodies positive, and it often persists after delivery.

  2. Very common

    Under-replacement in someone already treated

    Requirement rises early and by a substantial fraction. The commonest reason a treated woman's TSH climbs in the first trimester.

  3. Common

    Reading the result against a non-pregnant range

    Not a cause but a frequent misinterpretation in both directions. The pregnancy upper limit is lower, so ordinary ranges both over-reassure and under-reassure depending on the trimester.

  4. Common

    Iodine deficiency — in regions where dietary iodine is low

    Requirement rises in pregnancy, and intake that sufficed before may not now. Relevant for anyone avoiding iodized salt and dairy.

  5. Common

    Poor absorption of levothyroxine

    Pregnancy vitamins contain iron and calcium, both of which block absorption. Separating them from the thyroid tablet by four hours is the fix and it is rarely explained.

  6. Uncommon

    Hyperemesis with poor intake

    Severe vomiting interferes with taking and absorbing the tablet, and it coincides with the period of highest demand.

  7. Uncommon

    Postpartum thyroiditis in its underactive phase — in the year after delivery

    Often follows an overactive phase that went unnoticed. More likely with positive antibodies, and it may or may not resolve.
